We are still together in separate areas of the house and we have agreed to split bills during this time. For FL-150 #12, do I include her as a person living with me and show her helping in the household expenses? Also, do I include the bills she’s paying in #13 Average Monthly Expenses?

I personally find it easier and more helpful to fill out the form based on the current actual living situation. In this case, yes, you would include your spouse as a person living with you under #12. The ENTIRE household expenses (yes, including hers) would then be captured under #13. Under item #13s, you would total up the bills she is paying.
